Your First Visit
Welcome to our practice! We are thrilled that you entrust us for your dental needs.
Before your first visit, we encourage you to download and complete the Patient Registration, Medical History, and Dental History Forms to save you time completing these forms at your first appointment. If you choose to fill out the forms at our office, be sure to bring your contact information, dental insurance information, as well as your medical and dental history with you. The information is very important for us in order to initiate a personalized dantal care plan just for you. When you share those information with us, be sure to provide complete, up-to-date information on your health. Please Inform your doctor if you have had recent hospitalization or surgery. Please also let us know if you have recently been ill. We'd like to know the names, doses, and frequency of any medications you are taking, whether prescription or over-the-counter products, as well as the name of your physician. Please inform the doctor of any changes in your health or medications. This information will help the doctor to select the safest and most effective method of treatment for you.
Your first visit typically includes an x-ray that allows the doctor to view the structure of the jaw, the position of any teeth that have not yet erupted, malformed roots, and tooth decay.
With the help of a dental assistant, the doctor will conduct a comprehensive oral evaluation to assess your oral health. A complete treatment plan will then be developed with dental services the doctor recommends. In addition, the financial coordinator will meet with you to go over your dental benefit plan and discuss other financial options. Our scheduling coordinator will then work with you to schedule your next appointment.
